St. John's United Hospice / Home Social Worker Assistant in Billings, Montana
Monday – Friday 8am – 5pm
SUMMARY:
Home Health and Hospice provides home health and hospice care to elders residing in Yellowstone County. This position is located on the campus of St. John’s but is a joint venture partnership between Billings Clinic, Big Sky Senior Services and St. John’s United. The social worker assistant will be supervised by the MSW and agency Administrator. The social worker assistant will work within an interdisciplinary team to provide access to community resources, long-term care planning, financial resources and other appropriate social work interventions as necessary. The social work assistant will also be responsible for timely clinical documentation, attendance at team meetings and other administrative tasks as assigned.
ESSENTIAL JOB FUNCTION:
Provide living opportunities within nurturing environments of hope, dignity and love.
Assesses the psychosocial status of patients and families.
Carries out social evaluations and plans interventions or support services to patients and family/caregivers. Makes referrals as appropriate.
Assists with maintaining a bereavement program.
Maintains clinical records on all patients served.
Maintain hospice volunteer program in conjunction with St. John’s existing “no one dies alone” and “vigil care” programming.
Maintains collaborative relationships with organizational personnel and outside agencies.
Participates in discharge planning as appropriate.

Employee Responsibilities
Follows policy and procedures on TB testing.
Follows policy and procedures on annual influenza vaccinations.
Attends all mandatory in-services
Works under the general supervision of the Social Service Department
Has daily contact with the potential residents and their families
Has daily contact with nursing home direct care staff to communicate pertinent resident admission information
REQUIREMENTS: Minimum Requirements:
- Minimum of two years experience in health care, home health / hospice experience preferred.
Education Requirement:
Meet or exceed CMS/Medicare regulatory requirements for home health and hospice.
Bachelor’s degree in SW or other related field.
