SCL Health Pharmacist Clinical Ambulatory in Billings, Montana

Job Description:

The Clinical Pharmacist demonstrates consistent application of knowledge and skills to deliver appropriate, safe, efficient, effective, and cost effective pharmaceutical care in the scope of their particular assignment. This position is responsible for the care of patients as well as the development of effective, professional relationships within the multidisciplinary team.

Schedule

Monday-Friday generally 0800-1700 depending on clinic hours and scheduled patients/meetings.

There is potential for limited Work From Home (1 day per week) after minimum of 6mo after hire provided caregiver has met productivity goals and other requirements set forth.

ScopeAs an Ambulatory Clinical Pharmacist, you need to know how to:
Be responsible for the implementation of SCL Health Pharmacy Services and SCL Health Medical Group programs and services- Develop clinical pharmacy programs according to relevant policies and regulations- Review records of patients to determine the appropriateness of medication therapy and identify patients for program outreach. Evaluate patients conditions and any barriers to medication compliance to ensure all issues are being treated- Identify untreated health problems and refer patients to appropriate physicians or other clinical staff. Develop effective medication plans that minimize the risk of adverse side-effects- Provide medical staff with appropriate medication information for new and existing drug therapies to support safe prescribing practices. Assess the results of pharmaceutical treatments. Collaborate with healthcare professionals to ensure optimal patient care- Advise on the correct procurement, storage, and administration of drugs in the clinic setting- Prescribe and discontinue medications, adjust medication doses, and order labs in accordance with any governing collaborative practice agreements with authorization from the patients physician- Keep accurate documentation of medication plans and patient progress to meet requirements for efficient patient care and to support billing practices- Support efforts to continuously improve the quality of the services being provided through participation in the collection and analysis of pertinent data, and through the development of program modifications that are driven by the analysis.

This position is based out of our St Vincent Hospital department, but this is an outpatient, ambulatory clinical pharmacy position, working with patients from our primary care clinics in the greater Billings area as well as supporting clinics in Miles City, Absarokee, and Red Lodge remotely. It is not an inpatient position receiving referrals directly from inpatient providers, despite being physically located on the hospital campus.Minimum Qualifications

  • - PharmD Degree, required- Minimum of two (2) years of work experience in a pharmacy retail or ambulatory work setting OR completion of a pharmacy residency program, required

Preferred Qualifications

  • Experience in Ambulatory Clinical Pharmacy including working under collaborative practice agreement(s).
  • Strong interpersonal skills as this position is dependent on collaboration with the primary care clinics as well as working on our hospital-based outpatient clinical pharmacy team.
  • Experience working on ambulatory pharmacy related projects including quality improvement projects, provider education, and advocacy work to advance the ambulatory pharmacy profession
  • Experience using electronic means of communication/documentation within the medical field (i.e- electronic EMR, virtual meetings, Microsoft Teams chat, etc

Physical Requirements:

The hourly range for this position is listed below. Actual hourly rate dependent upon experience.
