WHAT YOU WILL BE DOING: AWARE is preparing to launch a new service and programming focused on expanding preventative and mental health services for youth and families. The pandemic wreaked havoc on health and wellness in families and access to services across Montana. Issues of significant concern include the increased incidence of abuse, neglect, domestic violence, and substance use. AWARE is excited to re-launch services into homes as the project will springboard clinical services and support into communities recovering from the pandemic fueled losses. As a FACT Team Child & Family Case Manager, you will provide services to identified children and families in order to assist in gaining access to needed medical, social, educational, vocational, recreational, and other services as requested or needed, utilizing AWARE's Wraparound philosophy. Caseload and/or billing expectations and requirements must be met on a monthly, quarterly, and yearly basis. Provides case management services to clients, collects data from collateral sources, reviews relevant records and other pertinent data and resources, participates as a member of a treatment team in the planning, formulation, and implementation and review of the Treatment, Personal Supports or Individual Service Plan, and provides and documents services as approved and directed.

ABOUT AWARE: AWARE is a statewide nonprofit organization that offers quality, community-based support for people with mental health and/or developmental disabilities and families with children ages 0 to 18. We serve all 56 counties in Montana. AWARE’s Mission: We help people live independent lives.

· We ensure people with mental health and/or developmental disabilities live in a home of their choice and provide the day-to-day support they need.

· We help adults with disabilities find fulfilling employment, either through an AWARE program or in the community.

· We support children ages 0-3 and their families through a number of early childhood programs in Billings, Bozeman, Butte and Helena.

· We ensure adults and children live successfully in their communities, providing home visits, help in school, assistance with services, psychiatric assessment, and outpatient counseling.

· We hire professional, experienced staff to serve the individuals we support, employing more than 800 employees across Montana.

A thorough understanding of human services, resources, and tools. Must have the ability to assess clients (and families) from a strengths model perspective, along with the skills, knowledge, and abilities to develop a theoretical perspective, in addition to the ability to work with a variety of people. Strong oral and written communication skills are a must.


A BS/BA Degree in a Human Services Related Field is preferred; an equivalency can be used based on documented work history, education, and volunteer experience.
