POSITION SUMMARY:

Responsible for providing behavioral services to One Health patients. Will utilize a balanced approach which includes perspectives of all disciplines involved in health services and will have a role definition flexible enough to accommodate the diversity of programs offered through the center.

ESSENTIAL JOB FUNCTIONS AND RESPONSIBILITIES:

  • Evaluates and treats patients/clients (including walk-ins) within licensed scope of practice; evaluates and treats mental illness and substance abuse, treats or appropriately refers patients, for specialty evaluation and treatment, provides preventive health/lifestyle advice and case management, and provides consultation to other staff.

  • Participates in multi-disciplinary, team-based care of patients/clients.

  • Provides consultation to other members of primary care and behavioral health care teams.

  • Participates in training, reporting, and other requirements of various grant supported programs implemented by One Health and partners.

  • Participates in the student and/or other clinical discipline training activities at the center.

  • Participates in the formation and evaluation of applicable protocols.

  • Maintains continuity of care when health center patients utilize other health care facilities, i.e. referrals, emergency room services, outpatient departments, specialty clinics, and inpatient services.

  • Provides documentation according to health center procedures and follows up on results.

  • Participates in staff meetings, including quality assurance and behavioral health audit activities in keeping with health center procedures.

  • Participates in on-site and off-site outreach and continuing education programs and ensures training to maintain license and to meet the needs of the population.

  • Participates in performance optimization and risk management activities as required.

  • Adheres to all One Health Policies and Procedures.

  • Cooperates and abides by all elements of One Health’s Compliance Program.

  • Provides exceptional customer service and models the mission of One Health when interacting with internal and external stakeholders promoting a positive image for the health center.

  • Other duties may be assigned as required or needed with or without prior notification.

    MINIMUM JOB QUALIFICATIONS:

    Education/Certificates/Licenses:

    Required

  • Master's Social Work (MSW) and licensed in the State of Montana or Wyoming as a LCSW, OR

  • Master's in Counseling and licensed in the State of Montana or Wyoming as a LCPC

  • BLS Certification upon hire

  • Valid driver's license

    Experience:

    Preferred

  • Two (2) years community health experience

    WORKING CONDITIONS AND ENVIRONMENT:

  • Work indoors in heat-controlled environment 95% of the time.

  • Moderate noise level consistent with an office environment.

  • Typically, 40-hour workweek.

  • Periodic travel to other One Health sites as well as regional and national meetings.
