JOB SUMMARY: Under the direction of the Director of Pharmacy, the Emergency Department (ED) Pharmacist is responsible for ensuring that safe, timely, and effective medication therapy is provided to patients receiving care at St. Peter’s Health Emergency Department and Urgent Care (UC) locations.
JOB DUTIES INCLUDE BUT ARE NOT LIMITED TO:
Verifies/enters medication orders and prepares/dispenses medications
Evaluates the appropriateness and effectiveness of the patient’s medications
Facilitates the integration of medical literature, clinical guidelines, and best practice to support clinical decision making and quality outcomes
Consults with providers in selecting medication therapy that best meets the needs of the patient and contributes to overall therapy goals
Manages pharmacy-driven collaborative drug therapy protocols
Responds to Code Blue, Trauma Alerts, and Stroke Alerts in the Regional Medical Center
Responsible for culture review and follow up for patients seen in ED or UC which includes consulting with providers to optimize therapy based on culture results, facilitating prescribing of appropriate therapy if indicated, and contacting patients to inform and educate on medication changes
Completes medication histories for ED patients
Serves as a preceptor for pharmacy residents and students in experiential rotations
Participates in multidisciplinary team meetings
Provides education on medications to patients, families, and staff
Documents medication interventions in the patient’s electronic health record and clinical decision support system
Works collaboratively with other pharmacy services to optimize care and the patient experience
