Logan Health Faith Community Nurse in Kalispell, Montana

Job Description Summary:

This position assesses and identifies patient needs per department procedure and protocol as applicable upon discharge and within the community setting.

Qualifications:

  • Current Montana RN license or a multi-state compact license with authorization to practice nursing within the state of Montana; required. BSN preferred.

  • Minimum of two (2) years’ RN experience in a similar position within a medical setting (e.g. physician office, clinic, or hospital) required. Minimum of one (1) year case management experience preferred.

  • Faith Community Nursing Certification preferred.

  • Healthcare Provider (HCP) CPR, ACLS, and/or BLS certification required and/or as dictated by each practice/service line.

  • Understanding of the confidentiality laws that govern the provider-patient relationship, including but not limited to HIPAA guidelines.

  • Excellent organizational skills, detail-oriented, a self-starter, possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.

  • Commitment to working in a team environment and maintaining confidentiality as needed.

  • Excellent verbal and written communication skills including the ability to communicate effectively with various audiences.

  • Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy.

  • Possess and maintain computer skills to include working knowledge of Microsoft Office Suite and ability to learn other software as needed.

Job Specific Duties:

  • Identifies clinical and social needs of patient related to admission and/or discharge. Contributes to the plan for transition in a safe and timely manner to the least restrictive environment that can meet each patient’s needs. Collaborates with stakeholders to address social determinants of health (SDOH) and a thorough admission and/or discharge plan as applicable to each assigned patient.

  • Identifies patients at risk for readmission and works with the interdisciplinary team across the continuum to formulate and implement a discharge plan to decrease the risk for readmission.

  • Acts as a resource to providers, patients, caregivers and interdisciplinary team regarding community resources.

  • Exhibits knowledge and skills necessary to provide for the spiritual, psychological, and cultural needs appropriate to the age, ethnicity and special needs of the patient. Acknowledges and advocates for a patient’s right to self-determination and informed consent.

  • Consistently reflects ownership and professionalism towards FCN. Identifies areas in need of improvement and serves an active role in developing solutions. Exhibits the Logan Health Core Values in all interactions.

  • Responds to, and provides direction in, a changing workload in a positive and professional manner.

  • Identifies and obtains necessary provider documentation and orders to ensure compliance with all applicable Logan Health, State, and Federal requirements as they relate to Utilization Review, Discharge Planning, and Care Coordination processes as applicable.

  • Communicates frequently with patient and family/support to promote the development of a patient-focused plan, and ensure that the patient and family/support members are informed and involved in decisions about care.

  • Serves as a Health Educator and utilizes evidence-based educational programs to promote healthy lifestyles and disease prevention, to foster stewardship of the body and whole person health.

  • Serves as a Health Counselor and Advocate. Assists individuals and families in their understanding of health issues and alleviates apprehension or concerns. May perform hospital, home, or nursing home visits. Empowers people to make informed decisions for care and care transitions, grounded in faith.

  • Performs as a Referral Agent with extensive knowledge of resources and community services to help with health needs.

  • Recruits and coordinates volunteers within the faith community to serve in various ministries. Develops of facilitates support groups to meet needs. Organizes educational health fairs and non-invasive health care screenings, e.g. blood pressure.

  • Maintains documentation as outlined in the ANA scope and standards of faith community nursing. Documents activities, maintains records and adheres to HIPPA privacy and confidentiality.

Logan Health operates 24 hours per day, seven days per week. Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed.
