Kalispell Regional Healthcare Inpatient Pediatric Social Worker, MSW in Kalispell, Montana
This position works in partnership with healthcare professionals to assure continuity and coordination of care for patients and families. Furnishes services and consultation to patients/families regarding social, environmental, psychological, financial and cultural factors related to illness and hospitalization. Supports continuity in case management and provides discharge management.
Masters of Social Work (MSW) required.
Strong background in Pediatric Social Work required.
Extensive knowledge of Social Work needs that are specific to the Pediatric population in relation to healthcare required.
Experience in Hospital Social Work preferred.
HS AED CPR must be successfully obtained within one (1) week of hire.
Excellent verbal and written communication skills including the ability to communicate effectively with various audiences. Excellent interpersonal skills with the ability to manage sensitive and confidential situations with tact, professionalism, and diplomacy.
Excellent organizational skills, detail-oriented, a self-starter, possess critical thinking skills and be able to set priorities and function as part of a team as well as independently.
*Conducts thorough psychosocial assessment of pediatric patients and families utilizing all available resources and according to applicable policies and regulations.
*Provides assistance with evaluation and referral in situations of possible abuse/neglect for pediatric patients.
*Documents assessment findings and social service interventions appropriately in the medical record and communicates same to the appropriate health care team members.
*Implements social service interventions based on psychosocial assessment.
*Provides pediatric patient and/or family counseling to assist with coping, grieving, conflict resolution, etc.
*Acts as an expert-resource to pediatric patients, family members, and the health care team regarding community and governmental resources.
*Works collaboratively and professionally with patients, family members, physicians, hospital staff and other stakeholders and agencies involved in providing pediatric patient care.
*Works collaboratively with case managers and the health care team in formulating and facilitating pediatric patients’ hospital discharge plans.
*Acts as a liaison between patients, family members, physicians, interdisciplinary team, and external agencies and organizations.
*Acts as a resource/advisor to physicians regarding social work issues that may affect pediatric patient care.
*Maintains knowledge and understanding of KRH policies and procedures, Medicare/Medicaid, managed care and other payer regulations and benefit limits.
*Develops and maintains professional relationships and knowledge of services, resources and providers specific to the pediatric population.
