Kalispell Regional Healthcare LPN Referral Intake Coordinator - Home Health in Kalispell, Montana


Provides intake information from referral sources and coordinates care for patients being referred to Home Options departments.


  1. *Responsible for receiving referral phone calls and faxes.

  2. *Accurately records patient and referral source information into current software.

  3. *Ability to use KRONOS consistently and accurately.

  4. *Gathers necessary information and ensures quality and timely communication between referral sources and admitting offices, including communicating pending admissions to department supervisor and/or scheduler.

  5. *Initiates appropriate communication with clinical, operational and financial staff based on the needs of the referred patient.

  6. *Ensures patient evaluations are performed accurately and timely by facilitating appropriate communication between clinical staff.

  7. *Performs initial assignment of evaluation visits, contact patient/family to notify of the service that will be provided. Coordinates initial plans to meet the immediate needs of the patient and family.

  8. *Completes hospital and nursing home rounds to identify potential referrals.

  9. *Assists with data collection regarding referral statistics, non-admits and "Not taken for Care."

  10. *Participates in the Face 2 Face process.

  11. *Educates personnel who accept referrals on the particulars of the Intake Process, specifically personnel associated with Home Options programs and products.

  12. *Serve as an ambassador to all Home Options departments and sites.


  1. Current BLS certification required.

  2. LPN with current state license preferred.

  3. Minimum one (1) year Home Health or Hospice experience preferred.

  4. Previous intake/scheduling experience preferred.

  5. Minimum one (1) year experience in telephonic triage/intake/disease management preferred.

  6. Strong telephone and customer service experience and ability to work effectively within a diverse team as well as independently. Must be proficient in Microsoft Outlook, Word and Excel.

  7. Working knowledge of Medical Terminology, Medicare/Medicaid and other insurance programs, regulations and guidelines regarding home health care. Excellent care planning and documentation skills.

  8. Licensed driver with a reliable and insured vehicle.
