Logan Health LPN - Skilled Nursing - Long Term Care in Kalispell, Montana

Job Description Summary:

This position follows a patient specific goal directed Plan of Care. Collaborates with clinical team and participates in the planning and evaluation of patient care. Services adult and at times, pediatric populations. Assists and supports the department team as needed.


  • Current Montana LPN license required.

  • Minimum of two (2) years’ experience in similar position with a medical setting (i.e. physician clinic or hospital preferred. Medical or surgical experience preferred.

  • Possess knowledge of professional nursing theory to give and evaluate patient care. Possess knowledge of basic skilled nursing procedures and medical terminology.

  • Current BLS certification preferred or must successfully obtain within sixty (60) days of hire.

  • Experience with equipment, medical supplies and wound care preferred.

  • Ability to make independent judgments under stressful situations at times. Must work effectively and efficiently without direct supervision.

  • Current Driver’s License required. Must possess a good driving record clear of moving violations in the past five (5) years and no major driving offenses (DUI, etc.).

  • Must have capability to connect to internet for daily data exchange.

Job Specific Duties:

  • Provides skilled nursing care to patients through the use of the nursing process as appropriate for LPNs in the homecare setting.

  • Contributes to maintaining an updated, individualized plan of care that includes; goals, interventions, medications and visit frequency orders.

  • Performs and documents physical, psychosocial and spiritual assessment of assigned patients utilizing all available sources of information. Evaluates and documents the patient’s perception of illness/condition.

  • Performs medication reconciliation during visits.

  • Responds to changing workload in a positive and professional manner, makes objective assessments, prioritizes and asks for assistance as appropriate.

  • Accurately and efficiently transcribes and implements physician orders and treatments, both verbal and written.

  • Consistently problem- solves issues affecting patient care and reports instances to leadership.

  • Maintains a thorough knowledge of infection control procedures and strictly adheres to policies and procedures.

  • Exhibits an understanding of and adherence to Home Health/Hospice CMS guidelines and Conditions of Participation.

Other duties may be assigned similar to the above consistent with knowledge, skills and abilities required for the job. Not all of the duties may be assigned to a position.

Maintains regular and consistent attendance as scheduled by department leadership.


Day Shift - 12 Hours (United States of America)

Logan Health operates 24 hours per day, seven days per week. Schedules are set to accommodate the requirements of the position and the needs of the organization and may be adjusted as needed.
