FERGUS COUNTY Disease Intervention Specialist in LEWISTOWN, Montana

Full or part time, permanent positionMust be willing to work variable hours, including early mornings, nights, and weekends as occasionally needed.WAGE: $24-$26/hrPOSITION SUMMARY:The Disease Intervention Specialist studies the occurrence of various disease and conducts investigations to prevent their spread. This position will coordinate investigations of communicable disease. This work includes conducting client interviews, case investigations, contract tracing, and collection of data for surveillance and reporting. This position will also assist with the development of appropriate response protocols, data tracking and analysis, and communication with stakeholders. Duties will also include responsibility for a variety of Public Health Emergency Preparedness (PHEP) grant deliverables. DUTIES:Coordinate Communicable Disease Surveillance Systems and Response. Conduct timely investigations with people with confirmed or suspected communicable disease. Manage patients with confirmed or suspected disease under investigation using established protocols including ongoing interviews and patient follow up, tracking of relationships/contacts, case management and documentation. Coordinate referrals to clinic settings for specimen collection and/or additional healthcare needs based on established protocol. Provide coaching, education, and advocacy in increase likelihood that health and safety protocols are following for improved individual health, as well as slowing and preventing the spread or disease of disease through the population. Communicate effectively and responsibly by establishing rapport, assertively confronting issues, and resolving patient concerns. Document surveillance data such as health indicators, risk factors, and disease surveillance information. Coordinate and manage special surveillance and screening activities during emerging disease events under established protocol and supervision. Monitor trends, risk factors, and prevention interventions or communicable disease in the county. Provide direct patient care: Testing and collection, Immunizations, Assist Family PlanningConducts CLIA waived testing QUALIFICATIONS:Registered Nursing degree or degree in Health Education required; baccalaureate degree in nursing or health education from an accredited university preferred.Must have a non-encumbered Montana current license to practice as a Registered Nurse in the State of Montana.Must have current Healthcare Provider CPR Certification.The job requires knowledge of physical, biological and behavioral sciences and general medical and patient care practices. Knowledge of educational and learning theories; public health funding sources and grant writing; health psychology theories; community assessments; and program implementation and evaluation.Must have skills and knowledge to operate computer and office equipment.Must have a valid Montana driver's license and a reliable vehicle.PHYSICAL REQUIREMENTS:Frequently required to lift and/or move up to 25 pounds without assistance of a co-worker.
