State of Montana DDP Targeted Case Manager (14407) in Missoula, Montana

The Developmental Disabilities Program procures and supports community-based services for people with developmental disabilities and/or their families. The DDP Targeted Case Manager is responsible for a comprehensive assessment and periodic reassessment of individual needs, the development of and periodic revision of the plan of care, referral and related activities to help the member obtain needed services, and monitoring and follow-up activities. The Case Manager is responsible to ensure the quality of the plan of care. If lacking one year’s experience in field of DD, must complete at least 40 hours of training in service delivery to persons with DD under a training plan reviewed by the Department within no more than 3 months of hire or designation as a case manager.
