If you are located in Alaska, Montana, Wyoming, Washington, Idaho, Oregon, Utah, North Dakota or South Dakota, you will have the flexibility to telecommute* as you take on some tough challenges.
Schedule regular follow-up for fee-for-service (FFS) Clients with appropriate clinical and non-clinical staff based on the Alaska’s specific FFS behavioral healthcare needs and as described in the plan of care.
Review assessments and assessed acuity level, diagnosis, and medical history and update the FFS Client’s information and plan of care at subsequent contacts with the FFS Client.
Engage with providers and behavioral health treatment teams in discharge planning and timely, successful transition to the community of Medicaid recipients.
Provide disease and intensive care management services for Alaska Medicaid beneficiaries.
Conduct telephonic and/or face-to-face independent assessment of the needs of Recipients determined to be eligible for the State Plan benefits to ensure Recipient’s needs are assessed and documented and to establish a plan of care. Conduct the independent assessments with the use of standardized assessment tools.
Develop person-centered service plans with Recipients, and/or their authorized representatives.
Evaluate members' ability to independently manage self and locate alternative resources when limitations are identified through a psychosocial assessment.
Assists recipients in accessing verified community, government, and/or client resources.
Facilitate member care transition through the healthcare continuum and refers treatment plans/plan of care to clinical reviewers as required
Participate in Quality Control and Process Improvement (QC/PI) activities.
Other duties as assigned.

Licensed Master's Degree in Psychology, Social Work, Counseling or Licensed Ph.D.
Licenses must be active and unrestricted in the state of residence
Must be willing to acquire Alaska license within 6 months of employment if an Alaska license has not already been obtained.
3+ years direct clinical experience in a behavioral health setting with emphasis on acute psychiatric or inpatient experience
Community behavioral health and/or public sector and Medicaid experience.
Proficient Microsoft skills (Word, Excel, Outlook) with the ability to toggle between multiple systems
A designated quiet workspace and access to install secure high-speed internet via cable/DSL in home
You will be provisioned with appropriate Personal Protective Equipment (PPE) and are required to perform this role with patients and members on site, as this is an essential function of this role
If you need to enter a work site for any reason, you will be required to screen for symptoms using the ProtectWell mobile app, Interactive Voice Response (i.e., entering your symptoms via phone system) or a similar UnitedHealth Group-approved symptom screener. When in a UnitedHealth Group building, employees are required to wear a mask in common areas. In addition, employee must comply with any state and local masking orders.
Residence in the Anchorage area is highly desired
Previous case management experience
Dual diagnosis experience with mental health and substance abuse
Knowledge of outcome focused, recovery-oriented clinical models and interventions in behavioral health
Strong communication and customer service skills
Previous experience working in Electronic Health System
